By clicking "Submit" I confirm I have NO signs or symptoms of a respiratory infection, such as fever, cough or shortness of breath, sore throat, muscle aches, runny nose, loss of taste or smell and I have NOT traveled to an area the CDC has designated as a Country/City, with High Risk exposure to spread Covid-19 in the past 14 days.

Additionally, I voluntarily assume all responsibility for my health (others in my party) and physical condition and certify that I am in good health. Upon arriving to God's Nation and God's Nation church events, I will follow all of the social distancing rules set forth, including, but not limited to, wearing a face mask/covering during the entire experience or event and following all individual protocols, as well as adhering to any additional protocols set forth by God's Nation, regardless of whether those rules are communicated in writing or verbally at any time before, during or after the event.
